Integumentary System: Skin Lesions

Skin lesions can include any abnormal area of the skin, so there are several terms to describe them. As a nursing assistant, you must let the nurse know if any new skin lesions appear and keep an eye on already existing ones for any changes in the appearance and size, increased pain, bleeding, and infections.
